Investigations Coordinator - Mosaic - Omaha, NE

We're looking for an Investigations Coordinator to join our team!

As an Investigations Coordinator, you’ll coordinate and perform complex internal investigations involving allegations of abuse, neglect, and exploitation of clients and any other investigations required by Mosaic policy and procedure.

What you’ll do in this role: 

  • Conducts internal investigations on a variety of topics as defined by organizational policy and procedure, including cases of abuse, neglect, exploitation, or mistreatment.
  • Monitor incidents and allegations that are or have the potential to meet the organizational definition of a sentinel event.
  • Communicates complex information of confirmed sentinel events based on protocol to designated teams 
  • Prepare relevant reporting  documentation as assigned in accordance with Mosaic’s Sentinel Event policy and procedure.
  • Develop scope and investigation strategy for investigations initiated in assigned regions.
  • Identifies, contacts, and interviews alleged perpetrator(s), witnesses, and victims.
  • Collect, collate, analyze, and interpret data relating to investigations assigned.
  • Lead multiple investigations at once in a fast-paced environment with high standards.
  • Evaluates, summarizes, and documents investigative findings. Prepares records, correspondence, and reports. Works with local operations to provide investigatory information to various federal, state, or local regulatory and/or law enforcement agencies.  
  • Present investigation findings to stakeholders in the company including agency leadership.
  • Assist in the development of investigations processes and policies
  • Participate in State regulatory investigation requests and the production of investigative materials in support of the latter.

SCHEDULE:  Monday-Friday 8:00 - 5:00 p.m.

SALARY:  $55,000 - $60,000

Cannot employ individuals in the following states:  Alaska, California, Hawaii, Nevada, Montana or Washington State

Ideally, you should have: 

  • Bachelor’s Degree in Business, Political Science, Human Services or related field preferred or equivalent experience required. 
  • Minimum of three years of related experience, with increasing responsibilities in an administrative or organizational environment. 
  • Experience in I/DD or other healthcare setting preferred. 
  • Certification in LRA Consulting’s Conducting Serious Incident Investigations / Level I or ability to obtain certification within twelve (12) months of hire date required. 
  • Master’s Degree preferred (MBA, MHA, JD, or similar).
